Dr. Lindsey A. George DDS & Lindsey Dentistry PLLC,
on an Out-of-Network, Non-Participating Relationship.

Claims will be initiated on your behalf by our office administrators for submission for any Out-of-Network benefits that you may be entitled to for your visit.

Payment of services will be due in full upon completion of each visit and any OON benefits would be mailed directly by the plan to the policy holder.

Dr. Lindsey A. George DDS & Lindsey Dentistry PLLC will be considered Out-of-Network after this date.

Delta Dental Insurance Companies
Dr. Lindsey A. George DDS's participation with All Delta Dental Companies and Networks will be resigned effective:
04/16/2023.
Dr. Lindsey A. George DDS & Lindsey Dentistry PLLC will be considered Out-of-Network after this date.

​Dr. Lindsey A. George and Lindsey Dentistry PLLC will still be able to continue to provide services
on a private pay basis to any and all Delta Dental patients that choose to continue your care at Lindsey Dentistry PLLC after this date.
Any Out-of-Network benefits that may be applicable with your plan will typically be mailed to you directly from Delta Dental after services are rendered.
We will continue to send your claims on your behalf as a courtesy.
The practice's full, usual & customary fees will be due at the time of service.
